by CareLink's housecall physicians. The medical needs of seniors are met by a collaborative team consisting of physicians that make housecalls along with their supporting nurse practitioners and physician assistants. This collaborative care approach offers seniors more choices and involvement in the care available to them while helping CareLink's physicians more effectively serve the needs of their senior patients. Physician housecalls increase access to and improve the quality of healthcare services for seniors. The main benefit for seniors and their families is the increased access and coordination of medical services offered in the safety and convenience of their home. By offering regularly scheduled medical housecall visits, CareLink's physicians are turning back the clock to a time when medical care was convenient and personalized. Medical housecalls includes both the treatment of acute care episodes as well as the ongoing monitoring of chronic illnesses; periodic health assessments that include screening, counseling, risk assessment, patient and family education; and the coordination and follow-up of referrals to and from other health care providers.
